Services

Second Chance Group Homes provides both in-home and community based services to children and adults with developmental disabilities and/or mental illness.

Habilitation Services

Habilitation Services are based out of the person’s own residence or a residence provided by SCGH. 

SCGH provides outcome-based training to enhance personal skill level in the home and community. Skill training is designed to increase or maintain an individual’s skills in the following areas:

• Daily living/Self help
• Social skills
• Active participation in the community
• Adaptive skills
• Cognitive and Motor Functioning
• Positive Communication

Staffing needs are based on each individual’s needs and individual service plans (ISP).

The goals of the HAH program are to empower the consumer to live an independent and productive life.

All care providers meet the State of Arizona requirements, have a valid Finger Print Clearance Card, and have successfully passed all of SCGH training courses and background checks.

Attendant Care

Attendant Care Services are provided in the participants’ own residence or family residence and allow an individual to remain in their own home by maintaining safe and sanitary living conditions both personally and environmentally. 

SCGH develops and facilitates a plan of care to ensure the highest quality of life for each participant. 

We do that by focusing on the following:

Daily life skills, including, but not limited to:
• Personal Hygiene
• Bathing, oral hygiene, toileting
• Attention to injuries or illness, and wound care
• Nutritional meal planning and grocery shopping
• Housekeeping
• Assistance with all Medications and reminders

All care providers meet the State of Arizona requirements, have a valid Finger Print Clearance Card, and have successfully passed all of SCGH training courses and background checks.

Respite Care

SCGH Respite Services are designed to provide quality care to individuals in the event that the primary care provider is unavailable. 

SCGH provides short- term relief to care providers and or guardians.
• Staffing is determined on a case-by-case basis based upon the client’s needs. One-on-one care is available.
• This service can be provided in various licensed community settings, group homes, developmental homes, nursing facilities and individual’s homes.
• Scheduled and emergency Respite services are available.

All care providers meet the State of Arizona requirements, have a valid Finger Print Clearance Card, and have successfully passed all of SCGH training courses and background checks.

Residential Services

Group Homes
SCGH has homes throughout the valley. Our homes offer a family-oriented, comfortable, safe and clean environment. Each client has their own private room.

SCGH strives to promote personal independence, healthy lifestyle choices and dignity while protecting individuals from harm.

Staff provide 24-hour care, 7 days a week, ensuring basic health & safety, personal hygiene, self-awareness, social, and community skills development.

We offer nicely decorated and landscaped home environments with up to five persons per home. SCGH encourages active involvement in the surrounding community.
We actively assist individuals in achieving personal accomplishments and becoming valued members of the community.

All homes are also monitored by local police and fire departments.

Areas in which our trained care providers assist clients include, but are not limited to:

– All Counseling, Medical and Legal Appointments
– Social outings, which include: sporting events, movies, outdoor activities and other recreational interests.
– Transportation to life events, emergency situations and assistance with personal care.
– Preparation of appropriate meals and snacks in accordance with client’s dietary and nutritional needs.
– SCGH has an open door policy. Family and friends are always welcome during visitation hours to stop by and visit our homes and residents.
